Payment Policies

• We accept cash, check, Visa, MasterCard and Discover
• All fees are based on the type of service provided.
• If the patient is a minor (18 years or younger), the parent or guardian is responsible for payment of the account in accordance with the policies outlined above.
• For elective or uncovered surgical services; all co-payments and deductibles are due before your surgery.
• If your insurance provider requires a referral for treatment, please make sure our office receives the referral before or on the day of your scheduled appointment.

Please remember that as a professional, your physician's first priority is your health and well-being; he or she will not recommend any unnecessary examination or treatment. However, should you have questions about the purpose or anticipated costs of a procedure, our staff will welcome the opportunity to fully discuss all aspects with you beforehand.
